More Details: Objective: To enhance the outcomes of surgical treatment for bilateral nephrolithiasis in children. Methods: We performed 64 open transverse-wedge lower pole resection of the kidney (LPRK), which we developed and introduced into practice on 28 patients on one side and 18 in stages on both sides. Results: A study examined the stone location in the pelvicalyceal system (PCS) in children with bilateral and unilateral nephrolithiasis. Out of 496 kidneys analyzed, stones were found in the renal pelvis in 426 cases (85.8%). In addition, stones were located in the lower calyx in 322 cases (64.9%), with 172 (53.4%) being present in both kidneys and 150 (46.6%) located in one kidney. Furthermore, stones were identified in the middle calyx in 172 cases (34.7%) and the upper calyx in 154 cases (31%). Conclusion: Transverse-wedge LPRK, an organ-preserving surgical approach for stones complicated by hydronephrosis, is the preferred method for treating nephrolithiasis in children. This technique aids in the prompt resolution of calculous pyelonephritis and helps prevent relapses of nephrolithiasis, which frequently necessitates repeated kidney surgeries that can lead to nephrectomy.
