Abstract (English): |
Epithelial tumours account for 98 % of all malignant breast tumours. The most frequent are common ductal types, lobular and particular types which often have very characteristic cytological aspects. The importance of treatment diagnosis for these particular types can be justified by an often better prognosis. Diagnosing malignancy can be confirmed by a cytological examination. Difficulties occur on the one hand when the atypias are discrete, and on the other hand in confirming the lobular variety of the proliferation. The in situ types can be suggered but it is always impossible to be precise about the cytological in situ or infiltrating character the proliferation. Carcinoma diagnosis in the pre- and post-partum period is often unambiguous ; the difficulty is to know well the physiological changes in the breast during this period so as not to wrongly diagnose malignancy. Carcinomas in man, essentially of the ductal type, can show all the cytological aspects of this tumor. The clinical presentation, in general, suggests malignancy with or without inflammatory cutaneous features, very different from the simple aspect of a gynaecomastia. [Copyright &y& Elsevier] |

Les tumeurs e´pithe´liales repre´sentent 98 % des tumeurs malignes du sein. On distingue les formes communes canalaires les plus fre´quentes, lobulaires, et les formes particulie`res dont les aspects cytologiques sont souvent tre`s caracte´ristiques. L'inte´reˆt du diagnostic pre´the´rapeutique de ces formes particulie`res se justifie par leur pronostic souvent meilleur. Le diagnostic de malignite´ peut eˆtre affirme´ sur un examen cytologique. Les difficulte´s surviennent d'une part lorsque les atypies sont discre`tes et d'autre part pour affirmer la varie´te´ lobulaire de la prolife´ration. Les formes a` de´veloppement in situ peuvent eˆtre sugge´re´es mais il est toujours impossible de pre´ciser cytologiquement le caracte`re in situ ou infiltrant de la prolife´ration. Le diagnostic des carcinomes de la pe´riode pre´- et post-partum est souvent sans ambigui¨te´ ; la difficulte´ est de bien connaiˆtre les modifications physiologiques du sein au cours de cette pe´riode pour ne pas porter a` tort un diagnostic de malignite´. Chez l'homme le carcinome, essentiellement de varie´te´ canalaire, peut prendre tous les aspects cytologiques de cette varie´te´ tumorale. La pre´sentation clinique est en ge´ne´ral e´vocatrice de malignite´ avec ou sans signes inflammatoires cutane´s, tre`s diffe´rente d'un simple aspect de gyne´comastie. [Copyright 2002 Elsevier] |
