Objective: Intraosseus lipomas are an uncommon pathological entity. Its prevalence, however, is believed to be underestimated. We present a clinical case seen in a medical center consultation. Clinical case: We report the case of a 57-year-old man without noteworthy pathological or family history, who consults at his health center due to periodic mild mechanical right heel pain during the past year. Results: A simple imaging study of the affected foot was requested, which revealed a well-defined osteolytic lesion with a thin, linear sclerotic margin and a central calcification, without cortical disruption or involvement of adjacent soft tissues, exhibiting a nonaggressive radiological appearance. This image is referred to as a "bull's eye" or "cokade" and is suggestive of an intraosseous lipoma. The diagnosis was confirmed with the performance of magnetic resonance imaging. Conclusion: The nonaggressive characteristics of this disease guides towards conservative management of intraosseus lipoma, reserving invasive surgical options only in case of complications or increased fracture risk. Objetivo: El lipoma óseo es una entidad patológica infrecuente. No obstante, se cree que su prevalencia se encuentra infraestimada. Reportamos un caso clínico visto en una consulta de un centro médico. Caso clínico: Se presenta el caso de un varón de 57 años, sin antecedentes médicos personales ni familiares relevantes, que consulta por presentar de manera periódica desde hace 1 año dolor leve de características mecánicas en el talón derecho. Resultados: Se solicitó estudio imagenológico simple del pie afecto en el que se evidenció una lesión osteolítica, bien definida, de aspecto radiológico no agresivo, de margen fino lineal esclerótico, con una calcificación central, sin disrupción cortical ni compromiso de partes blandas adyacentes. Esta imagen se denomina bull's eye (ojo de buey) o cokade (escarapela) y es sugestiva de lipoma intraóseo. El diagnóstico fue confirmado con la realización de una resonancia magnética. Conclusión: Las características no agresivas de esta patología encaminan el quehacer médico hacia a un manejo conservador, reservando la opción quirúrgica en caso de complicaciones o de un incremento del riesgo de fractura.
