The document discusses the validation of the Revised-2 International Staging System (R2-ISS) for overall survival in multiple myeloma patients within the Balkan Myeloma Study Group (BMSG). The R2-ISS incorporates serum LDH, del17p, t(4;14), and +1q21 aberrations to classify patients into four risk categories. The study compared R-ISS and R2-ISS in a real-world cohort of 1,503 patients, showing significant differences in overall survival across stages II and III and stages III and IV. The R2-ISS was found to better identify high-risk patients, especially within the intermediate-risk group, suggesting its potential for tailoring treatment strategies and prioritizing patients for clinical trials.
